Upload Your AHA HeartCode BLS Online Portion Certificate

Upload the certificate from your AHA eLearning account showing that you completed the HeartCode® BLS Online Portion.

Your certificate must clearly show:

Your name
The words “HeartCode® BLS Online Portion”
The phrase “has successfully completed”
The completion date
The certificate number
The American Heart Association logo or AHA branding

Important: Do not upload a receipt, payment screenshot, class registration confirmation, BLS Provider eCard, CPR/AED/First Aid certificate, ACLS/PALS certificate, or any unrelated document.

If you do not have this certificate yet: Log in to your AHA eLearning account, open your completed HeartCode BLS course, download the certificate, and return to this form when you have the correct file.

Rejection warning: Documents that are incorrect, incomplete, blurry, unrelated, expired, altered, or missing the required information will be rejected and will need to be resubmitted before your paperwork can be processed.

Upload Your Current or Recently Expired AHA BLS Provider Card

Upload your American Heart Association BLS Provider eCard or wallet-card PDF showing that you currently hold, or recently held, an AHA BLS Provider certification.

Your card/document must clearly show:

Your name
“BLS Provider”
American Heart Association or AHA branding
Issue Date
Renew By date
eCard Code or QR code
The card must be currently active or expired within 30 days of your class date

Full-page AHA BLS Provider eCard PDF
AHA BLS Provider wallet-card PDF
Clear image or screenshot of the AHA BLS Provider card

Important: Do not upload a receipt, payment screenshot, class registration confirmation, HeartCode certificate, CPR/AED/First Aid certificate, ACLS/PALS certificate, hospital badge, or unrelated document.

If you need to find your AHA card: Go to the AHA card lookup page at www.heart.org/cpr/mycards, search for your card, then download or screenshot the correct AHA BLS Provider card.

Rejection warning: Documents that are incorrect, incomplete, blurry, unrelated, altered, expired beyond the allowed window, or missing the required information will be rejected and will need to be resubmitted before your paperwork can be processed.
