Required Document: ACLS Precourse Self-Assessment Completion

Upload your ACLS Precourse Self-Assessment completion certificate from the AHA.

Your document must clearly show:

  • Your full name
  • "Advanced Cardiovascular Life Support Precourse Self-Assessment"
  • That you passed the self-assessment
  • Completion date

Do NOT upload: PALS paperwork, HeartCode certificates, Precourse Self-Assessment and Video Prework (that is for initials only), provider cards, receipts, login pages, quiz screenshots, or unrelated documents.

Renewal students: You must also submit a current AHA ACLS Provider Card (active or expired within 30 days). If you have not uploaded it yet, submit this document first, then return to this form and submit your card separately.

Required Document: Current AHA ACLS Provider Card

Upload a clear photo or scan of your current AHA ACLS Provider Card.

Your card must clearly show:

  • Your full name
  • "ACLS Provider" or "Advanced Cardiovascular Life Support"
  • American Heart Association
  • Issue date and Renew By date
  • Card / eCard code

The card must be active or expired within the last 30 days.

Do NOT upload: PALS cards, BLS cards, expired cards beyond 30 days, completion certificates instead of the card, or unrelated documents.

Required Document: ACLS Precourse Self-Assessment and Video Prework Completion

Upload your ACLS Precourse Self-Assessment AND Video Prework completion certificate from the AHA.

Your document must clearly show:

  • Your full name
  • "Advanced Cardiovascular Life Support Precourse Self-Assessment and Video Prework"
  • That you passed the self-assessment AND completed the video prework
  • Completion date

Do NOT upload: PALS paperwork, HeartCode certificates, the Precourse Self-Assessment only version (that is for renewals), provider cards, receipts, or unrelated documents.

This is the required document for initial ACLS students.

Required Document: HeartCode ACLS Online Portion Completion Certificate

Upload your HeartCode ACLS Online Portion completion certificate from the AHA.

Your document must clearly show:

  • Your full name
  • "HeartCode ACLS Online Portion"
  • That you successfully completed the online portion
  • Certificate number and completion date

Do NOT upload: PALS HeartCode, Precourse Self-Assessment certificates, provider cards, receipts, or unrelated documents.

HeartCode is only for hands-on skills sessions.

Which one do you need?

ACLS Renewal Prep Work is for students whose AHA ACLS Provider card is currently active OR was expired within 30 days of your class date.

ACLS Initial Prep Work is for students taking ACLS for the first time, OR whose AHA ACLS Provider card has been expired for more than 30 days before your class date.
